Chang and Gravenstein Join Staff Jewish Family Service Association is pleased to announce the addition of specialized Geriatric Medical Assessment Services at Alyson’s Place – JFSA Medical Clinic. These specialized services will assist older adults who are experiencing symptoms of dementia or other cognitive disorders, including Alzheimer’s, related to aging. Services will be offered onsite at the clinic or in the home. With the launch of these services, JFSA welcomes two nationally recognized physicians to Alyson’s Place. Dr. Stefan Gravenstein, MD, MPH, received his Medical Degree from The Ohio State University and his Master in Public Health from Eastern Virginia Medical School. He is the Director of the Center for Geriatric and Palliative Care at University Hospitals-Case Medical Center and the Interim Chief of Medicine for the Geriatrics and Palliative Care in the Department of Medicine at Case Western Reserve University. Dr. Gravenstein is a Certified Medical Director and Professor of Medicine at Case Western Reserve School of Medicine and adjunct Professor of Medicine and Health Services as Brown University. He was the founding medical director of memory disorders clinics at both the University of Wisconsin-Madison and Eastern Virginia Medical School, where he served nearly 20 years. Dr. Sandy Chang, MD, MHS, received her Medical Degree at Brown Medical School with a specialization in family medicine. She completed her Certificate in Gerontology at the Center of Aging and Health at Johns Hopkins Bloomberg School of Public Health. Dr. Chang comes to University Hospitals from Yale University. She is a 2011 Hartford Scholar in Geriatric Medicine and conducts clinical research on the epidemiology of aging, including the impact of chronic disease on frail elders. Dr. Chang is an Attending Physician at University Hospitals Case Medical Center, Acute Care of the Elderly Unit, and Assistant Professor at the Center for Geriatrics and Palliative Care at Case Western Reserve University School of Medicine. Dr. Chang will lead the clinical services including comprehensive geriatric assessments and practical advice for family members regarding issues facing those living with dementia. Dr. Chang and Dr. Gravenstein will also provide hands-on dementia training to JFSA’s Care at Home staff including Home Health Aides. Alyson’s Place is a collaboration with University Hospitals designed to provide JFSA clients and their family members convenient access to quality healthcare services.
